Comprehensive care for women's health is one of the foundations of the Unified Health System (SUS) and requires care that recognizes changing needs throughout the life course, overcoming the historical concentration of assistance on the pregnancy-puerperal cycle. This study aimed to analyze how Primary Health Care (PHC) and the Family Health Strategy (FHS) organize and provide comprehensive care to women in Brazil, identifying barriers, inequalities, and organizational factors related to access, longitudinality, and care coordination. This is an integrative literature review, covering the period between 2016 and August 2026. The search report used as a basis retrieved records in Elicit and PubMed, which were subjected to screening and full-reading stages. For this synthesis, 15 studies with the greatest adherence to the object, thematic diversity, and relevance to the Brazilian PHC context were selected. The findings were organized into five axes: permanence of the maternal-reproductive focus; expansion of care throughout the life course; violence, diversity, and inequities; territory, coordination, and PHC organization; and multiprofessional team and FHS. The results show advances in coverage of maternal and reproductive actions, but discontinuities in the puerperium, gaps in mental health and aging, barriers related to violence and sexual diversity, territorial inequalities, and coordination failures with specialized services persist. The performance of community health workers and the territorial organization of the FHS appear as relevant elements for the quality of care. It is concluded that comprehensiveness remains partially realized and requires expanding the scope of care, qualifying teams, strengthening network coordination, and equity-oriented strategies.
