Aging is associated with physical and functional changes that may compromise autonomy and quality of life in older women. In this context, the Pilates method has been investigated as a strategy to improve different components of physical performance. This study aimed to analyze the effects of the Pilates method on functional capacity and quality of life in older women. This integrative literature review was based on controlled clinical trials identified through searches in PubMed/MEDLINE, the Virtual Health Library (VHL), and SciELO databases, supplemented by reference screening. A total of 156 records were identified in the databases, along with five additional studies, resulting in the inclusion of 13 independent clinical trials. The findings demonstrated predominantly favorable effects on functional autonomy, balance, mobility, muscular strength, flexibility, and aerobic endurance. However, benefits varied according to intervention protocols, assessment instruments, and participant characteristics. Regarding quality of life, improvements were observed in specific domains, although findings were less consistent across studies. Some comparison interventions produced similar or superior benefits in certain functional components. Methodological heterogeneity and small sample sizes limited comparisons between studies. It was concluded that the Pilates method represents an alternative for improving functional capacity in older women and may contribute to maintaining autonomy. Nevertheless, its effects on quality of life remain variable, highlighting the need for further studies using standardized protocols to clarify the magnitude and consistency of the benefits observed in this population.
